The New York State Workers’ Compensation Board is adopting a national standard for the first report of injury which will eliminate the C-2 form. For Flanders Group clients with the New York State Insurance Fund please discontinue using the C-2 and the Supervisors’ Accident Report for reporting Workers’ Compensation claims effective immediately.
To report future claims:
1. Click here https://www.flandersgroup.com/client-service-center/report-a-wc-claim/ to go to our website where you can find the new First Report of Injury Worksheet.
2. Send us the completed form and we will enter it into the State Insurance Fund system for you. That will generate an immediate claim number which will be forwarded to you.
None of the other Workers’ Compensation forms (C-240, C-11, or C-107) have changed.
